Alabamians who have had COVID asked to take survey by ADPH

The Alabama Department of Public Health is asking Alabama residents who have had the COVID-19 virus to take a confidential survey. 

The survey is to help the department investigate the spread of the virus and with contact tracing efforts in the state. All answers to the survey are confidential and will take five to 10 minutes to complete. 

The link to the survey can be found here or by going to alabamapublichealth.gov and clicking on the “Have you had COVID-19?” banner.
